If there are different contacts for various departments, please provide the relevant contact information for each. • If the Trust utilizes ÃÛÌÒÓ°Ïñ Professionals (ÃÛÌÒÓ°ÏñP) or an equivalent master vendor/neutral vendor, please include the contact details (name, job title, email address, and phone number) of the individual(s) responsible for onboarding third-party suppliers to the Trust via these vendors. | |

| Answer To Question 2: 2. N/A - Currently there is no process for applying to be on the PSL, as the contract has only been in place for 6 months we wish to give the suppliers who successfully tendered for the opportunity the chance to deliver on the trust's requirements. This will be something we review later in the contract's life. | |
