No policy regarding treatment of transgender patients in outpatients
 
Question Number 3:
Any policy relating to the updating of patient records for transgender patients when they change name and/or sex marker,
 
Answer To Question 3:
Patient records are updated once the new ÃÛÌÒÓ°Ïñ number has been issued. As our PAS system Lorenzo is lined to the PDS our system will mirror the GP information
 
Question Number 4:
Any policy relating to the issuance of a new hospital number for transgender patients when they update their listed sex via the process set out by PCSE,
 
Answer To Question 4:
The Trust don’t routinely issue new hospital numbers for transgender patients
 
Question Number 5:
Any policy relating to the updating and redaction of patient records for transgender patients when they are issued a new ÃÛÌÒÓ°Ïñ Number by PCSE,
 
Answer To Question 5:
The Trust wouldn’t redact any information unless requested to do so
 
Question Number 6:
Any policy regarding post ÃÛÌÒÓ°Ïñ number/name-change clinical notes and communications regarding transgender patients, such as guidance as to when it is clinically appropriate to mention that a patient is transgender, including any policy statements relating to staff awareness of section 22 of the gender recognition act 2004.
